A clearer starting point

Care with a little more context.

You do not need to arrive with the right words. We look at the pattern, explain what we find and shape practical next steps around the life you actually lead.

01

Listen closely

We begin with the moments that feel limited, not a prewritten script.

02

Map the pattern

A grounded assessment gives you a useful picture of how the pieces are working together.

03

Build your next move

Your plan is clear, teachable and designed to travel beyond the treatment room.

Ways we can work together

Specific care. Everyday relevance.

Choose a starting point, bring your questions and leave with a better sense of what comes next.

01 / FIRST VISIT

Movement assessment

A focused first session to understand your current concern and the movement around it.

02 / ONGOING

Hands-on care

Thoughtful sessions that support mobility and help you stay connected to your changing needs.

03 / PRACTICE

Movement coaching

04 / RETURN

Active-life support

A measured path back to the activities, work and routines that matter to you.

“The goal is not to chase a perfect body. It is to give you more options.”

A note from Dr. Mara Vale

Good care makes the next decision easier. Horizon Form is a small practice built around clear explanations, attentive hands and the belief that progress should feel usable when you walk back out into your day.

Before you visit

Good questions welcome.

What happens at a first appointment?

We talk through your concern, review relevant history, assess how you move and explain the useful options we see. You will have time for questions.

Do I need a referral?

No referral is needed for a first conversation. If your situation calls for another kind of care, we will say so clearly.

What should I wear?

Wear comfortable clothing that lets you move. You do not need special equipment or preparation.

Is chiropractic care right for my concern?

That depends on the concern and your context. An assessment is a chance to understand whether this approach is appropriate before deciding on a plan.

How often will I need to come in?

There is no preset schedule. We recommend the lightest useful approach and revisit it as your movement and goals change.

Can I ask questions before booking?

Absolutely. Use the assessment request below and mention what you would like to understand. We will reply with the next practical step.
